FDA's MedWatch Online Voluntary Reporting Form
Use the MedWatch form to report adverse events that you observe or suspect for human medical products, including serious drug side effects, medication errors/product use errors, product quality problems, and therapeutic failures. ECLA encourages reporting of contact lens complications to the FDA via the MedWatch online voluntary reporting form for medical devices. Alerting the FDA to complications is an important mechanism by which laws, regulations, guidelines, and labeling can be updated to improve the public health and safety.
